I had the same issue with my daughter's fire and I think it is related to storage. It's almost as if the app doesn't have enough room to download so it fails, but it never provides a message that there is a storage problem. One of the previous suggestions was to select "Remove from Device" and do a hard restart, but that is not an option only "Remove from Carousel" which leads me to believe that it does not fully download.
A hard restart also did nothing to help. I removed the app from the carousel, then removed a bunch of apps, and restarted. The problem was resolved.
Now whenever her Fire starts acting glitchy I reduce her apps and make sure she has plenty of storage. Or alternate method to clear FreeTime's apps data. Sign up to join this community. If you do elect to install apps or games currently installed on your profile in your child's FreeTime applications through the FreeTime settings and those apps or games are currently installed on an SD card, there will be some heartburn, as the apps will likely either not install in your child's profile, or they will temporarily install only to become magically uninstalled in the near future.
The best approach to doing that is to move the apps you want to install in your child's Amazon FreeTime profile back to be installed on your device's local storage. After that, then opt to install those applications to your child's FreeTime profile. After those applications have successfully been installed in your child's profile, you can safely move the apps or games back to being stored on the SD card. Because the Amazon FreeTime interface for your child is accessed through an app on the Kindle Fire tablet, you can always access the regular tablet interface for an adult experience.
